Understanding Human Error: Beyond Assumptions and Misconceptions
Human error is often viewed through a pessimistic lens, where it is seen as an inevitable occurrence linked to carelessness or a lack of attention. This perspective leads many to believe that the only remedy is rigorous training or punitive measures, such as job loss. However, a more nuanced understanding reveals that human error is not simply a product of individual failings, but rather a complex interaction with increasingly intricate systems.
As technology evolves and systems become more sophisticated, the likelihood of errors tends to rise. Individuals operating these systems often navigate a maze of complexities that can obscure their understanding, making mistakes more probable. This underscores the importance of designing systems that minimize reliance on perfect human performance. A historical example is the near-miss incident at the Three Mile Island nuclear reactor, where a series of human errors compounded an already critical situation. These included unrecognized equipment failures and incorrect operational decisions, highlighting the need for better system design to support human operators.
When analyzing human error, it is helpful to categorize mistakes into two main types: errors of omission and errors of commission. Errors of omission occur when a critical task or step is overlooked. In contrast, errors of commission involve incorrect actions, such as selecting the wrong control or executing tasks in the wrong order. These classifications help identify the nature of the mistakes and can inform strategies for error prevention.
Engineers are particularly interested in quantifying human behavior to better predict errors. This effort has led to the concept of human error probability (HEP), which attempts to estimate the likelihood of errors occurring in various contexts. While predicting mechanical and electrical failures has its controversies, human error predictions are even more complex due to the subjective nature of human behavior. Despite the challenges, HEP is being employed in numerous fields, particularly within the military, where crew resource management is critical during operations.
Recognizing that human error is often a symptom of a flawed system rather than an inherent flaw in individual operators can shift the focus toward improving system design. By creating environments that account for human limitations and enhance overall safety, we can work towards reducing the impact of human errors and fostering a more reliable operational framework.
